This is a golfer called ddetts on Golfwrx who has been providing a diary of events in the evolution of his swing (and other aspects of his golf game).

Here is his swing over a year ago and then he decided to attend Monte Scheinblum's clinic.
Here is his swing approx 1 year later
What do you think he needs to improve on considering (over the course of the year) he has had to have a hip replacement and also experiencing left knee issues too (not good for such a relatively young man)?
Some points I've noticed in his current swing (have I got this correct?):
1. At setup his right hand grip seems overly weak.
2. His takeaway is moving too far inside.
3. His chin is tucked tight on his chest - won't that impede the rotation of his left shoulder in the backswing . Won't that in turn prevent him from efficiently weight loading onto his right hip/leg at the top of the backswing so that he can utilise his pelvic rotary muscles in the hip squaring phase of the downswing?
4. His right leg straightens and doesn't seem to retain any knee flex (maybe relating to point 3 above)
5. He seems to be releasing PA#2 early in the downswing (maybe too much PA#1) and is using a punch right elbow action rather than an optimal:
a. Right arm adduction , while right shoulder moving down plane
b. Pitch elbow and right lateral tilt .
6. He is running out of right arm and using an non-optimal hand release action through impact (flipper sub-type of non-DH hand release action?).
He does have a strange transition at P4 , as if his left forearm is pronating slightly more as he starts his downswing- or could he be 'regripping'?
